Auditions - Teacher Information

Thank you for considering recommending some of your current students for membership in The St. John's Boys' Choir. SJBC depends on the support of local teachers each year to let us know about boys who would be good candidates for membership. This collaboration has gone on successfully for 31 years and we are so grateful that it continues.

On this page, you can download a recommendation form. From this point you have four (4) options:

1. Print the form. You can fill in the form by hand and then mail the form to SJBC.
2. Fill in the form electronically (complete online or save to your computer and complete offline). Print the form and then mail to SJBC.
3. Fill in the form electronically (complete online or save to your computer and complete offline). Save the revised form to your computer and then email the filled-in PDF to SJBC.
4. Fill in the form electronically (complete online or save to your computer and complete offline). Click the "Submit by Email" button to use your email client to send an .xml file (which contains the information you entered) directly to SJBC.

Please note that the form has room for 12 recommendations. You are free to submit as many recommendations as you would like. Simply use additional forms to send in extra names. If you teach at multiple schools, please use a separate form for each school.
